CC: boredom and looking for adventure and major life change
History of Present Illness:
The subject (aka patient) is a 28 year old lady with no known preexisting medical conditions, who is upon after finishing medical school found herself 'lost' and cannot seem to find herself. There is no associated internal conflicts whatsoever except for occasional bouts of laziness and feelings of wanting to become a bum but at baseline she is the type who wants to keep herself busy with various activities. She then found herself jumping from 1 job to the next landing 12 different jobs in three months for which she found herself just reporting for work for 2-3 days and then not going back again. She later then found herself just going to the mall, the gym, spa and coffee shops. She then later relaized that 'this can't go on forever', hence the dilemma and the need for a solution.
Past Medical History
unremarkable and noncontributory
health buff at heart but does not work out much, loves to walk
no history of mental illness
no PMD
Personal and Social History
nonsmoker
occasional alcoholic beverage drinker
denies and is strongly against illicit drug use
certified coffee addict
certified shopaholic
naive at times
Patient is from the very large continent of Asia. Can speak English (no Cyaracom interpreter needed). Single, no children. Currently employed and finished some post graduate studies.
Review of Systems:
(+) anorexia
no loss of appetite
(+) occasional headaches
(+) occasional sleep disturbance
(+) occasional malaise
Family History
noncontributory
Physical Examination
Alert female, in no apparent distress, appears 'younger than state age' (ahemm...)
VS: stable
HEENT: WNL
Heart and Lung: WNL
Abdomen: NABS, soft, nontender
Ext: No edema, no cyanosis
Assessment/Plans
A 28 year old lady who has a very minor dilemma of not knowing what to do after finishing medical school. She was contemplating on whether to stay in her country and proceed with residency training or go somewhere else where she could also do residency. Other problems to consider are what program to finally take.
Plan: Go to the United States
Take the MLEs
Apply and start residency and at the same time live the life she wants and the lifestyle she wants.
